  • To say yes or no to services or medications.
  • To access your records and information.
  • To file a complaint or appeal without fear of retaliation.
Confidentiality & Privacy
  • Your personal information is protected by law (HIPAA).
  • We only share information with your consent or when required by law.
  • Records are kept secure and confidential.
Safety & Incident Reporting• Our staff are trained to keep you safe at all times.
  • If something unsafe happens (like an accident or injury), it will be reported right away.
  • You or your guardian will always be informed about incidents and our follow-up.
Non-Discrimination

  • Services and opportunities are available to all individuals equally.
Grievances & Complaints

  • We will acknowledge your complaint within 72 hours and resolve it within 30 days.
  • Filing a complaint will never affect your ability to receive services.
Staff Training & Competency
